Below is a outline of the key eligibility criteria for our Housing Stabilization Services.

1Age Requirement
Adults and Elderly Individuals: Services are available to adults aged 18 and older, including elderly individuals who require support to maintain or transition into stable housing.
2Disability Status

Individuals with Disabilities: Eligibility extends to those who have a documented disability that impacts their ability to maintain stable housing. This includes physical, mental, developmental, or emotional disabilities.

Medical Documentation: Applicants must provide medical or psychological documentation of their disability, which may include a diagnosis from a licensed healthcare provider.

3Housing Instability Risk

Risk of Homelessness: Individuals who are at risk of becoming homeless, including those currently experiencing housing instability or residing in unsafe or temporary housing, are eligible for services.

Homelessness: Those who are currently homeless or have recently experienced homelessness are also eligible for assistance.

4Minnesota Residency

State of Minnesota: Individuals who are at risk of becoming homeless, including those currently experiencing housing instability or residing in unsafe or temporary housing, are eligible for services.

5Enrollment in Medical Assistance (Medicaid)

Medical Assistance Enrollment: Eligible individuals must be enrolled in Minnesota’s Medical Assistance (Medicaid) program. Housing Stabilization Services are part of the benefits available under this program.
